The nerve to quadratus femoris and inferior gemellus arises from the anterior divisions of L4, L5, and S1, passing through the greater sciatic foramen below the piriformis, deep to the sciatic nerve. It descends anterior to the gemelli and obturator internus to enter the deep surface of quadratus femoris, also supplying the inferior gemellus and the hip joint.

Clinical Notes

The nerve to quadratus femoris provides articular branches to the posterior hip capsule, making it a target for image-guided nerve blocks for posterior hip pain. Ischiofemoral impingement syndrome involves compression of the quadratus femoris and its nerve between the lesser trochanter and the ischial tuberosity, producing deep posterior hip pain with walking. MRI demonstrates oedema within the quadratus femoris in ischiofemoral impingement. The nerve is at risk in posterior hip approaches and hamstring origin surgery.
