The deep head provides DRUJ compression — a function analogous to the rotator cuff at the shoulder. AIN palsy denervates PQ, producing subtle pronation weakness.

Clinical Relevance

Clinical Notes

In distal radius fractures, PQ is elevated for volar plate fixation and must be repaired at closure to restore DRUJ stability. AIN palsy assessment includes specific PQ testing.

Palpation

Tested by resisted pronation with elbow at 90 degrees to eliminate pronator teres.

Detachment of PQ deep layer compromising DRUJ stability, requiring repair at the time of volar plate fixation.
