The thoracic multifidus provides segmental control of thoracic spinal motion, spanning 2-4 vertebral levels. It is less developed than lumbar multifidus but key to thoracic posture.

Thoracic multifidus dysfunction contributes to thoracic kyphosis in osteoporosis. In scoliosis, asymmetric thoracic multifidus hypertrophy is found on the convex side.
