The anterior head of the deltoid is the flexion and internal rotation component. It works antagonistically to the posterior head during shoulder rotation. The anterior head is the most active during pressing movements.

The anterior deltoid head is divided in the deltopectoral approach to the shoulder — the interval between anterior deltoid and pectoralis major is the deltopectoral groove (cephalic vein). This approach is used for shoulder arthroplasty, Latarjet, and proximal humeral fracture fixation.
