The vesical fascia is the visceral peritoneal reflection and the surrounding condensed connective tissue enclosing the urinary bladder, continuous with the umbilicovesical fascia superiorly and the pubocervical or puboprostatic fascia inferiorly.

Encloses the bladder and maintains its position within the pelvis, provides the surgical plane for extraperitoneal cystectomy, and contains the bladder lymphatics and neurovascular supply.

Clinical Relevance

Clinical Notes

In radical cystectomy, the correct surgical plane follows the vesical fascia — too superficial risks entering the bladder or leaving tumour, too deep may injure the rectum or uterus. In pelvic organ prolapse repair, the vesical fascia is plicated to support the anterior vaginal wall.

Vesical Fascia Plane in Radical Cystectomy

Dissection within the vesical fascia plane during cystectomy maintains the correct tissue plane, complete oncological resection, and protection of adjacent structures.

Vesical Fascia Defect in Anterior Compartment Prolapse

Defect in the vesical fascia allowing bladder descent through the anterior vaginal wall (cystocele), repaired by anterior colporrhaphy or mesh augmentation.
