Transverse Acetabular Ligament Detail

ligamentum transversum acetabuli detail

The transverse acetabular ligament bridges the acetabular notch, converting it into a foramen through which the acetabular branch of the obturator artery and acetabular neurovascular structures pass. It completes the inferior acetabular rim.

Region: Pelvis & Hip
Biomechanics

Function & Actions

Completes the inferior rim of the acetabulum, provides the inferior attachment for the acetabular labrum, and serves as the primary surgical landmark for acetabular cup position during total hip arthroplasty.

Clinical Relevance

Clinical Notes

The transverse acetabular ligament is the most important intraoperative landmark for acetabular cup inclination — the cup should be positioned parallel to the TAL (approximately 40 degrees inclination in most patients). The ligamentum teres of the hip attaches to the TAL at its midpoint.

Pathology

Common Injuries & Conditions

Transverse Acetabular Ligament as Cup Inclination Reference

The TAL serves as the intraoperative reference for acetabular cup inclination in total hip arthroplasty — parallel cup placement to the ligament produces consistent and reproducible cup angles.

TAL Disruption in Acetabular Fracture

Transverse acetabular ligament injury in acetabular wall and column fractures affecting the inferior acetabular rim, assessed during ORIF planning for inferior stabilisation requirements.
