TMJ Capsule

The TMJ capsule is a fibrous sleeve enclosing the entire joint, attaching superiorly to the articular eminence and glenoid fossa and inferiorly to the neck of the mandibular condyle. It is reinforced laterally by the temporomandibular ligament.

Function & Actions

Contains the synovial fluid that lubricates both joint compartments, limits excessive jaw movement, and provides proprioceptive feedback about jaw position.

Clinical Relevance

Clinical Notes

TMJ capsule inflammation (TMJ capsulitis) produces diffuse joint pain worsened by jaw movement. Capsular tightness following trauma or surgery restricts mouth opening. In total TMJ replacement, the capsule is released to allow implant placement.

TMJ Capsulitis

Capsular inflammation producing diffuse periauricular pain and restricted opening, managed by NSAIDs, warm compresses, and occlusal splint therapy.

Capsular Fibrosis

Post-traumatic or post-surgical capsular scarring producing trismus, treated by aggressive physiotherapy and progressive jaw stretching.
