TFCC Peripheral Dorsal Attachment

insertio peripherica dorsalis complexi fibrocartilaginei triangularis

The dorsal peripheral attachment of the TFCC connects the dorsal margin of the triangular fibrocartilage to the dorsal rim of the sigmoid notch and the dorsal DRUJ capsule, providing the dorsal stabilisation of the complex.

Region: Forearm
Biomechanics

Function & Actions

Provides the dorsal anchor of the TFCC to the distal radius, resists dorsal translation of the ulnar head, and contributes to DRUJ stability in concert with the volar attachment.

Clinical Relevance

Clinical Notes

Peripheral dorsal TFCC tears (class 1B by Palmer classification) are repairable — they heal because this region is vascularised. Arthroscopic outside-in repair of the dorsal peripheral TFCC tear restores DRUJ stability. MRI arthrography best demonstrates peripheral TFCC tears.

Pathology

Common Injuries & Conditions

Peripheral Dorsal TFCC Tear (Palmer 1B)

Vascularised peripheral dorsal TFCC avulsion producing DRUJ instability and ulnar-sided wrist pain, repaired arthroscopically with outside-in suture technique.

TFCC Repair in DRUJ Instability

Peripheral dorsal and volar TFCC repair as the primary surgical treatment for traumatic DRUJ instability from TFCC avulsion.
