The anterior superior tibiofibular ligament connects the anterior fibular head to the anterolateral tibial condyle, forming the anterior constraint of the proximal tibiofibular joint.

Primary anterior constraint of the proximal tibiofibular articulation, resists anterior fibular head displacement during external tibial rotation.
Proximal tibiofibular dislocation from high-energy knee injury or a fall on a flexed knee disrupts the anterior ligament. The common fibular (peroneal) nerve wraps around the fibular neck just distal to this joint and is at risk from displacement or traction. MRI shows ligament discontinuity and periarticular oedema at the fibular head. Chronic symptomatic instability may need surgical stabilisation by ligament reconstruction or temporary fixation.
