The superior acromioclavicular ligament reinforces the superior aspect of the AC joint capsule, contributing to horizontal AC joint stability alongside the inferior AC ligament. Together the superior and inferior AC ligaments form the complete AC joint capsular reinforcement. The superior ligament is the component typically palpated during AC joint assessment.

The superior AC ligament is specifically targeted in AC joint prolotherapy and PRP injection for AC joint osteoarthritis and Grade I-II separation. Its thickening on ultrasound is an indicator of AC joint degeneration alongside inferior AC joint space osteophytes.
