The anterior sternoclavicular ligament reinforces the anterior capsule of the SC joint, running from the anterior-superior clavicular head to the anterior manubrium, forming a broad flat band.
The primary restraint against anterior SC dislocation and superior clavicular displacement, provides anterior joint stability, and is the ligament disrupted in the more common anterior SC dislocation.
Anterior SC dislocation (more common than posterior) produces a prominent clavicular head medially. Most reduce spontaneously or can be reduced closed and managed conservatively despite frequent residual prominence. The anterior SC ligament is tightened in figure-of-eight ligament reconstructions for chronic anterior SC instability.
Anterior clavicular head displacement producing a visible medial prominence, typically managed conservatively as posterior neurovascular structures are not at risk.
Figure-of-eight tendon graft reconstruction for chronic anterior SC instability failing conservative management.
