The quadrangular membrane is the upper portion of the laryngeal fibroelastic membrane, extending from the lateral epiglottis and aryepiglottic fold margin to form the free lower edge that produces the vestibular (false) vocal fold.
Forms the lateral wall of the vestibule, produces the false vocal cord by its free inferior margin, and provides the elastic support for the aryepiglottic fold during swallowing.
Supraglottic carcinoma spreads along the quadrangular membrane — lateral tumour extension to the paraglottic space involves this structure. In supraglottic laryngectomy, the cuts are made through or just below the quadrangular membrane. Acute supraglottitis causes massive edema of the quadrangular membrane producing the characteristic clinical appearance.
Lateral extension of epiglottic or aryepiglottic carcinoma through the quadrangular membrane into the paraglottic space, upstaging tumour to T3.
