Pubocervical Ligament

The pubocervical fascia (ligament) is the fascial condensation connecting the cervix to the pubic bones anteriorly, providing the primary support for the anterior vaginal wall and the bladder base. Its defects produce anterior vaginal wall prolapse (cystocele) that allows the bladder to herniate into the vagina. Paravaginal repair restores the lateral pubocervical fascial attachments.

Anterior compartment pelvic organ prolapse (cystocele) is the most common form of prolapse, resulting from pubocervical fascia defects either midline or lateral (paravaginal). Anterior vaginal wall repair restores the fascial support, and native tissue or mesh-augmented repairs are performed depending on severity and recurrence risk.
