Pterygospinous Ligament

ligamentum pterygospinosum

The pterygospinous ligament connects the lateral pterygoid plate to the spine of the sphenoid, occasionally ossifying to form the pterygospinous foramen through which the buccal nerve passes.

Region: Head & Skull
Biomechanics

Function & Actions

Provides structural support between the pterygoid plate and sphenoid spine, and when present as a bony bar, creates an additional foramen in the infratemporal fossa.

Clinical Relevance

Clinical Notes

The pterygospinous bar (when ossified) may trap the buccal nerve, producing cheek numbness or sensory alteration. It is identified in infratemporal fossa surgery and in the planning of mandibular nerve blocks.

Pathology

Common Injuries & Conditions

Pterygospinous Bar Buccal Nerve Entrapment

Ossified pterygospinous ligament compressing the buccal nerve producing cheek numbness, identified on CT scan of the infratemporal fossa.
