Proximal Lateral Patellar Retinaculum

The proximal lateral patellar retinaculum extends from the lateral border of the patella proximally to the iliotibial band and the lateral intermuscular septum at mid-thigh level. It is the superior extent of the lateral retinacular system and transmits forces from the IT band to the patella, contributing to lateral patellar tilt and traction in patients with IT band tightness.

Clinical Notes

The proximal lateral retinaculum is released in lateral release procedures for patellar maltracking when distal release alone is insufficient. Excessive IT band tightness transmits through this structure to produce lateral patellar compression syndrome. MRI demonstrates thickening and signal change in the proximal lateral retinaculum in chronic lateral patellar compression. Proximal realignment procedures must address this structure alongside the distal retinaculum.

Excessive lateral traction on the patella via the proximal and distal lateral retinacula producing lateral facet chondropathy and pain, associated with IT band tightness and vastus medialis weakness.
