The posteromedial corner (PMC) of the knee includes the posterior oblique ligament, semimembranosus expansions (including the oblique popliteal ligament), posterior medial capsule, and the posterior horn of the medial meniscus. It provides posteromedial rotational stability.
Resists posteromedial tibial rotation, limits valgus stress with the MCL, constrains posterior tibial translation medially, and provides the posteromedial capsular restraint that must be addressed in complete medial knee reconstructions.
PMC injuries accompany high-grade MCL injuries and ACL/PCL combined injuries. The PMC is assessed by the posteromedial drawer test and valgus stress at 0 degrees. Repair of the posteromedial corner in combined MCL-PMC injuries improves medial stability beyond MCL repair alone.
