The posterior tibiotalar capsule is the posterior wall of the ankle joint, reinforced by the posterior tibiotalar components of the deltoid and PITFL. It is the site of posterior ankle impingement from capsular fibrosis or synovitis.

Provides the posterior ankle joint boundary, limits excessive ankle plantarflexion, and is the capsule impinged between the posterior tibia and calcaneus during forced plantarflexion.

Clinical Relevance

Clinical Notes

Posterior ankle impingement syndrome from posterior capsular hypertrophy or os trigonum produces pain during forced plantarflexion — common in ballet dancers. Posterior ankle arthroscopy in prone position addresses posterior capsular pathology through posteromedial and posterolateral portals.
