A fibrous periosteal band reinforcing the deep transverse leg fascia against the posterior tibial periosteum, providing additional structural support to the deep posterior compartment in the lower leg. It contributes to the fascial compartment boundary.

Clinical Relevance

Clinical Notes

This periosteal band is divided during deep posterior compartment fasciotomy for compartment syndrome, and its integrity contributes to the compartment boundary that elevates pressure in acute compartment syndrome. It is also the region stripped during tibial intramedullary nail placement where the posterior periosteum is elevated by the nail's endosteal passage.
