Posterior labral tears (reverse Bankart lesions) occur in posterior shoulder instability from recurrent posterior loading — common in offensive linemen and gymnasts. The Kim lesion (posterior inferior labral tear with posteroinferior labrocapsular separation) is a specific pattern causing subtle posterior instability.
