The posterior intermuscular septum separates the medial adductor compartment from the posterior hamstring compartment along the medial femur. It provides structural compartmentalisation of the posteromedial thigh and defines the medial boundary of the hamstring compartment.

The posterior intermuscular septum is breached in posterior compartment haematomas tracking from hamstring tears and in deep thigh infections. It defines the medial boundary of the posterior thigh dissection during orthopedic and oncological surgery. Isolated posterior compartment syndrome of the thigh is uncommon but recognised after severe hamstring crush injury.

Bleeding from a hamstring tear tracking within the posterior fascial compartment, contained by the posterior septum and producing a tense, painful thigh swelling.
