The superior peroneal retinaculum connects the lateral malleolus to the lateral calcaneus, forming the roof of the retromalleolar peroneal groove and constraining the peroneal tendons behind the fibula.

Clinical Notes

SPR disruption from inversion injury or direct trauma allows peroneal tendon subluxation, producing a snap on dorsiflexion. Surgical repair or groove deepening with retinaculum repair restores tendon constraint.
