Peroneus Longus Plantar Tunnel Ligament

The peroneus longus plantar tunnel ligament is the fibrous band at the cuboid notch that anchors the peroneus longus tendon as it turns 90 degrees from the lateral to the plantar foot, creating the fibro-osseous tunnel constraining the tendon at this critical angle.

Anchors the peroneus longus tendon at the cuboid groove as it redirects from lateral to plantar, maintains tendon position during foot movement, and creates the tunnel through which the os peroneum (sesamoid) slides.

Clinical Relevance

Clinical Notes

Peroneus longus tenosynovitis at the cuboid tunnel produces lateral plantar midfoot pain. Os peroneum fractures occur from the constrained peroneus longus tendon within this tunnel. Tunnel release addresses peroneus longus impingement at the cuboid.
