The palmar radioulnar ligament is the deep TFCC component connecting the volar sigmoid notch to the ulnar fovea, providing the primary volar DRUJ stabiliser and resisting dorsal ulnar head displacement during supination.

Function & Actions

Resists dorsal ulnar head subluxation during supination, provides the volar constraint of the DRUJ, and forms the palmar floor of the TFCC complex.

Clinical Notes

Palmar radioulnar ligament tears allow dorsal ulnar head prominence in supination — the most common DRUJ instability pattern. MRI arthrography demonstrates the tear at the foveal attachment. Repair via arthroscopic foveal reattachment restores the primary volar constraint.
