The posterior capsule is the first structure to fail in posterior elbow dislocation — it tears from the olecranon as the joint dislocates. In elbow contracture release, the posterior capsule is excised arthroscopically or open to gain terminal extension. Posterior capsular calcification produces posterior elbow pain and limited extension.
