The mesoappendix is the triangular peritoneal fold attaching the appendix to the terminal ileum mesentery, containing the appendicular artery (a branch of the ileocolic artery) and appendicular vein.

Provides the vascular supply to the appendix via the appendicular artery, suspends the appendix from the terminal ileum, and provides the surgical pedicle divided during appendectomy.

The mesoappendix is divided during appendectomy — laparoscopically using an endoscopic stapler or bipolar energy. The appendicular artery tip (often the last vessel before the appendix tip) must be included in the division to prevent post-operative bleeding. In gangrenous appendicitis, the mesoappendix becomes thickened and friable, requiring careful haemostasis.

Gangrenous appendicitis producing mesoappendix oedema and necrosis, requiring careful division with attention to the fragile tissue to avoid inadvertent vascular injury.
