Medial Patellotibial Ligament

ligamentum patellotibiale mediale

The medial patellotibial ligament connects the medial inferior patella to the medial tibial plateau, providing secondary medial patellar restraint in deep knee flexion positions where the MPFL is less taut. It is typically repaired alongside the MPFL in patellar stabilisation procedures targeting the medial retinacular complex.

Region: Knee
Anatomical Data

Origin, Insertion & Supply

OriginMedial patellar border (inferior to the MPFL)
InsertionMedial proximal tibia
Biomechanics

Function & Actions

ActionsSecondary medial patellar stabiliser below the MPFL; resists lateral patellar displacement in deep knee flexion
Clinical Relevance

Clinical Notes

The MPTL is assessed and repaired as part of the comprehensive medial retinacular reconstruction for recurrent patellar dislocation in procedures targeting the full medial retinacular complex beyond the MPFL alone.

Pathology

Common Injuries & Conditions

Patellar Instability Medial Complex

Combined MPFL and MPTL disruption in recurrent patellar dislocation requiring comprehensive medial retinacular reconstruction.
