Posterior Medial Elbow Capsular Ligament

ligamentum capsulare posterius mediale cubiti

The posterior medial elbow capsular ligament reinforces the posterior UCL bundle and restricts posteromedial elbow impingement. It becomes contracted in valgus extension overload syndrome producing limited elbow extension and osteophyte formation in the posteromedial olecranon.

Region: Upper Arm
Biomechanics

Function & Actions

Posterior medial elbow containment; limits excessive valgus in deep flexion; supplementary to the UCL posterior bundle.

Clinical Relevance

Clinical Notes

The posterior medial capsular ligament is released in elbow extension contracture surgery and addressed during posteromedial impingement debridement. Contracture of this structure limits terminal elbow extension in throwing athletes and after elbow dislocation.

Pathology

Common Injuries & Conditions

Posterior Medial Capsular Contracture

Posterior medial capsule fibrosis limiting elbow extension in throwing athletes from repetitive valgus loading, managed by arthroscopic posterior medial capsule release as part of posteromedial impingement surgery.
