The lumbosacral ligament is a strong fibrous band connecting the inferior transverse process of L5 to the lateral sacral ala, effectively tethering L5 to the sacrum and reinforcing the L5-S1 facet capsule.

Function & Actions

Resists excessive L5 mobility on the sacrum, reinforces the iliolumbar ligament system at the L5-S1 level, and contributes to lumbosacral junction stability — the level subjected to the greatest biomechanical stress in the lumbar spine.

Clinical Relevance

Clinical Notes

The lumbosacral ligament is at risk during L5-S1 surgery, particularly in interbody approaches requiring mobilisation of L5. Its disruption contributes to post-operative lumbosacral instability. The superior L5 nerve root runs directly beneath this ligament and can be compressed by lumbosacral ligament hypertrophy.
