The long radiolunate ligament runs from the ulnar radial styloid to the lunate, providing secondary restraint against lunate ulnar displacement and complementing the RSC ligament's sling function. Together with the RSC it forms the volar extrinsic ligament framework that prevents carpal dislocation. The space of Poirier lies between them.

The long radiolunate ligament is preserved in proximal row carpectomy — the lunate fossa articular surface of the radius must articulate with the capitate head, and the long radiolunate origin provides the ligamentous bridge that is repaired at the completion of the procedure to provide some carpal constraint.
