The ligamentum flavum at the C1-C2 level is the posterior capsular ligamentous complex between the C1 and C2 laminae, providing the posterolateral C1-C2 joint closure and yellow ligament equivalent at this transitional level.

Provides elastic posterior C1-C2 constraint, assists return of the atlantoaxial joint to neutral after rotation, and forms the posterior boundary of the epidural space at C1-C2.

Clinical Relevance

Clinical Notes

Hypertrophy of the C1-C2 ligamentum flavum equivalent contributes to upper cervical canal stenosis. In posterior C1-C2 fusion, the interlaminar ligament complex is removed to allow bone graft placement. The C1-C2 epidural space at this level is used for upper cervical epidural injection.
