The ligament of Humphrey is the anterior meniscofemoral ligament, running from the posterior horn of the lateral meniscus anteriorly to the medial femoral condyle, coursing anterior to the PCL.
Provides an additional posterior lateral meniscal attachment to the femur, supplements the posterior cruciate ligament as an accessory restraint, and stabilises the posterior lateral meniscal horn against excessive anterior translation.
The Humphrey ligament is present in 50-70% of individuals and must be identified during PCL reconstruction — it can be mistaken for a PCL remnant or the PCL itself. In PCL repair surgery, the Humphrey ligament may provide partial constraint even when the PCL is torn.
The anterior meniscofemoral ligament must be distinguished from the PCL during arthroscopic PCL surgery to avoid inadvertent resection of this secondary PCL support structure.
