The accessory band of the forearm interosseous membrane (proximal oblique cord/Weitbrecht ligament) is a short oblique fibrous band in the upper forearm running from the lateral ulna to the radial tuberosity region, perpendicular to and proximal to the main membrane.
Resists proximal radial migration, transmits longitudinal force from the radius to the ulna at the proximal forearm level, and provides the proximal accessory band to the central interosseous membrane.
The oblique cord resists proximal radial migration in Essex-Lopresti injuries — its disruption along with the central interosseous membrane allows progressive radial shortening. In forearm interosseous membrane reconstruction, the oblique cord anatomy guides graft placement for the proximal radioulnar relationship.
Loss of the proximal oblique cord in interosseous membrane disruption contributing to proximal radial migration after radial head excision, requiring radial head arthroplasty.
