The inferior pulmonary ligament is a double-layered pleural fold connecting the inferior medial lung to the mediastinum below the pulmonary hilum, providing the inferior tethering of each lung.
Anchors the inferior lung to the mediastinum, stabilises the lung position during respiratory movement, and is a surgical landmark separating the inferior mediastinum from the inferior pulmonary vein.
Division of the inferior pulmonary ligament is a routine step in lung resection and pneumonectomy — it allows the remaining lung to expand and fill the space, and provides access to the posterior mediastinal lymph nodes at station 9. The inferior pulmonary vein lies just superior to the ligament origin.
Routine step in lobectomy and pneumonectomy, allowing mobilisation of the lung and access to inferior mediastinal lymph node stations 8 and 9 for staging.
