The iliopsoas bursa is the largest bursa in the body, lying between the iliopsoas tendon and the hip capsule, communicating with the hip joint in 10-15% of individuals through a defect in the anteroinferior hip capsule.

Reduces friction between the iliopsoas tendon and the hip joint capsule during hip flexion, may communicate with the hip joint serving as a safety valve for excess joint fluid, and becomes distended in hip joint pathology.

Clinical Relevance

Clinical Notes

Iliopsoas bursitis from hip OA, RA, or iliopsoas snapping produces anterior hip or inguinal pain. When communicating with the hip joint, bursal distension may produce a palpable groin mass (iliopsoas bursa cyst in hip OA). Ultrasound-guided aspiration and injection addresses symptomatic bursitis.

Hip joint effusion tracking into the communicating iliopsoas bursa producing a large groin mass in hip osteoarthritis, managed by joint aspiration and treatment of the underlying hip disease.

Snapping Hip Tendon Over Bursa

Internal snapping hip from iliopsoas tendon snapping over the iliopectineal eminence against the bursa wall, producing an audible anterior groin click treated by physiotherapy or tendon release.
