The inferior band of the iliofemoral ligament runs from the anterior inferior iliac spine to the intertrochanteric line inferior to the superior band, converging to form the base of the Y shape.

The inferior iliofemoral band is at risk during anterior approaches to the hip — the Smith-Petersen and direct anterior approaches require careful identification of this band to avoid inadvertent capsular damage that predisposes to post-operative instability.
