Inferior Glenohumeral Ligament — Anterior Band

The anterior band of the IGHL is the primary static restraint against anterior glenohumeral instability when the arm is in the abduction-external rotation (ABER) position of the late cocking phase of throwing. It is the ligament torn in anterior shoulder dislocations. Bankart lesion is the avulsion of the anterior IGHL anterior band from its glenoid labral origin.

Clinical Notes

The anterior IGHL anterior band is the primary structure repaired in Bankart repair for anterior shoulder instability — arthroscopic suture anchor reattachment of the anterior labrum and IGHL to the anterior inferior glenoid restores the anterior bumper and IGHL origin. The apprehension test in ABER position reproduces instability by stressing the anterior IGHL.
