The hyoepiglottic ligament connects the anterior surface of the epiglottis to the superior border of the hyoid bone body, maintaining the epiglottis position relative to the hyoid during swallowing.

Tethers the epiglottis anteriorly to the hyoid, limits inferior epiglottic descent during swallowing, and maintains the pre-epiglottic space between the epiglottis and hyoid bone.

Clinical Relevance

Clinical Notes

The pre-epiglottic space (between the hyoepiglottic ligament and the thyrohyoid membrane) is a potential space for supraglottic cancer spread — tumour invasion of this space upstages supraglottic tumours. In supraglottic laryngectomy, the hyoepiglottic ligament is divided and the resection passes through the pre-epiglottic space.

Pre-epiglottic Space Invasion via Hyoepiglottic Ligament

Supraglottic carcinoma invading the pre-epiglottic space by extending through the hyoepiglottic ligament, upstaging the tumour to T3 and requiring total laryngectomy or definitive chemoradiation.
