Posterior labral tears are less common than anterior but occur in posterior hip impingement during extreme extension and external rotation (dance, gymnastics). In posterior hip dislocation, the labrum may be avulsed from the posterior rim, contributing to post-reduction instability.

Posterior hip impingement from ischiofemoral compression or posterior bony impingement producing posterior labral tears in dancers and gymnasts, managed by arthroscopic labral repair.

Posterior labrum detachment accompanying posterior hip dislocation, contributing to recurrent instability if not repaired at the time of fracture-dislocation surgery.
