The anterior hip capsule in the iliofemoral zone is the thick anterior wall of the hip joint reinforced by the iliofemoral Y-ligament, creating the strongest region of the hip capsule and the primary anterior restraint against femoral head escape.

Anterior capsular laxity from hip impingement surgery (over-aggressive capsulotomy) produces anterior hip instability — the hip dislocates anteriorly in extension-external rotation. Capsular closure after hip arthroscopy preserves this restraint. Femoroacetabular impingement bone resection combined with capsular closure reduces post-operative instability risk.
