The fibular sesamoid ligament connects the lateral (fibular) sesamoid to the lateral FHB head tendon, the lateral MTP capsule, and the adductor hallucis transverse head insertion.

Provides the primary lateral constraint for the fibular sesamoid, maintaining its position within the first MTP plantar complex during toe motion and push-off.

Fibular sesamoid displacement is the more common sesamoid injury in hallux valgus — the adductor pull and valgus deformity lateralise the fibular sesamoid from its groove beneath the first metatarsal head.
