DRUJ Volar Capsular Ligament

The volar DRUJ capsular ligament is the palmar component of the distal radioulnar joint capsule, reinforced by the volar radioulnar ligament of the TFCC. It runs from the volar ulnar head to the sigmoid notch of the radius.

Function & Actions

Resists dorsal ulnar head displacement during supination, stabilises the DRUJ in the supinated position, and is the primary constraint against ulnar head dorsal subluxation.

DRUJ instability from volar capsular ligament disruption produces dorsal ulnar head prominence in supination. In TFCC reconstruction for DRUJ instability, the volar capsular ligament is tightened or reconstructed using a tendon graft. Volar capsular laxity is the primary lesion in many DRUJ instability patterns.

Dorsal ulnar head subluxation during supination from volar DRUJ ligament insufficiency, managed by TFCC repair or volar capsulorrhaphy.

DRUJ Instability in Distal Radius Malunion

Distal radius malunion altering sigmoid notch geometry disrupting volar DRUJ capsular tension, contributing to chronic DRUJ pain and instability requiring corrective osteotomy.
