Dorsal DRUJ Capsular Ligament

ligamentum capsulae posterioris articulationis radioulnaris distalis

The dorsal DRUJ capsular ligament is the posterior component of the distal radioulnar joint capsule, running from the dorsal rim of the sigmoid notch to the dorsal ulnar styloid base. It resists volar subluxation of the ulnar head during supination and is the primary dorsal stabiliser of the DRUJ.

Clinical Notes

The dorsal DRUJ capsular ligament is injured in Smith fractures with dorsal DRUJ instability, allowing volar ulnar head subluxation in supination. Its integrity is assessed during DRUJ stress testing under fluoroscopy. Repair or reconstruction is required in chronic dorsal DRUJ instability. It forms the dorsal component of the TFCC structural ring.
