The anterior tibionavicular band is the most anterior portion of the deltoid ligament, running from the anterior medial malleolus to the navicular bone and spring ligament complex. It is the first deltoid component torn in progressive eversion ankle injuries.
Anterior medial ankle stability; resists eversion and external rotation of the talus; first component stressed in lateral ligament sprains with eversion forces.
The tibionavicular band contributes to the medial ankle stability complex with the spring ligament for prevention of planovalgus deformity. Its disruption in severe ankle fractures contributes to talar tilt and plafond incongruity. MRI arthrography of the ankle identifies anterior deltoid tears that may require repair in ankle fracture-dislocation management.
Tibionavicular band disruption in high-energy ankle injury producing medial gutter widening and talar tilt, contributing to post-traumatic planovalgus and requiring repair as part of comprehensive ankle reconstruction.
