Costovertebral Joint Capsules

capsulae articulationum costovertebralium

The costovertebral joint capsules enclose the articulations between the rib heads and the adjacent vertebral bodies and intervertebral discs, providing the fibrous joint containment reinforced by the radiate and interarticular costovertebral ligaments.

Region: Chest
Biomechanics

Function & Actions

Contain the costovertebral synovial joints, allow the gliding rotation of the rib heads during breathing, are reinforced by the radiate ligaments, and are the capsules involved in costovertebral joint arthritis.

Clinical Relevance

Clinical Notes

Costovertebral joint dysfunction produces paraspinal thoracic pain reproduced by trunk rotation and deep breathing, mimicking thoracic disc disease. Fluoroscopic-guided costovertebral joint injection is both diagnostic and therapeutic. In posterior thoracic tumour resection, the costovertebral joint capsules are released during costotransversectomy.

Pathology

Common Injuries & Conditions

Costovertebral Joint Arthritis

Degenerative costovertebral joint capsule changes producing posterior thoracic paraspinal pain reproduced by deep breathing and trunk rotation, managed by targeted injection.

Costovertebral Joint Capsule Release in Costotransversectomy

Division of the costovertebral joint capsule during posterior thoracic tumour resection or thoracic disc approach via costotransversectomy, allowing rib removal for lateral vertebral body access.
