Carpal Boss Ligament

ligamenta articulationis carpometacarpi (carpale protuberans)

The dorsal capsular ligaments at the second and third CMC joints at the carpometacarpal ridge, forming the base of the carpal boss (os styloideum or CMC osteophyte).

Region: Hand & Wrist
Biomechanics

Function & Actions

Provides dorsal CMC joint stability at the index and middle metacarpal bases.

Clinical Relevance

Clinical Notes

The carpal boss (os styloideum) is a bony prominence at the second-third CMC junction, either a separate ossicle or an osteophyte from degenerative CMC arthritis. The overlying dorsal capsular ligaments are involved in the chronic pain from this anomaly. Excision of the boss with ligament repair is the surgical treatment.

Pathology

Common Injuries & Conditions

Carpal Boss Causing Dorsal Wrist Pain

Dorsal CMC joint ligament hypertrophy and osteophyte formation producing a painful prominence at the second-third CMC junction, managed by corticosteroid injection or surgical excision.
