Radiocapitellar Capsular Ligament

ligamentum capsulare radiocapitellare

The radiocapitellar capsular ligament is the thickened lateral portion of the elbow joint capsule that reinforces the annular ligament in resisting radial head lateral displacement. It is disrupted in lateral elbow dislocation and in the LUCL tear that produces posterolateral rotatory instability.

Region: Upper Arm
Biomechanics

Function & Actions

Radial head containment within the capitellotrochlear groove; lateral elbow joint containment; supplementary to the annular ligament for radial head stability.

Clinical Relevance

Clinical Notes

The radiocapitellar capsular ligament is assessed during elbow arthroscopy by probing the lateral capsule for laxity. In posterolateral rotatory instability (PLRI), the capsular ligament and LUCL allow the radial head to rotate posterolaterally on the capitellum during the pivot shift test. LUCL reconstruction addresses this capsular ligament simultaneously.

Pathology

Common Injuries & Conditions

Radiocapitellar Capsule Disruption in PLRI

Lateral capsular ligament tear allowing posterolateral radial head subluxation on the capitellum producing a positive pivot shift test, managed by LUCL reconstruction restoring lateral elbow constraint.
