Clinical Notes

CFL injuries rarely occur in isolation and almost always accompany ATFL tears in moderate-severe sprains. Talar tilt test assesses combined integrity.

CFL Sprain

Inversion in neutral or dorsiflexion tearing the CFL, producing posteroinferior malleolar tenderness and positive talar tilt — virtually always combined with ATFL injury in Grade III sprains.
