The broad ligament is a double-layered peritoneal fold draping over the uterus, fallopian tubes, and ovaries like a tent, dividing the pelvic cavity into anterior and posterior compartments. It contains the parametrium, round ligament, ovarian ligament, fallopian tube, and ovarian vessels.
Provides the peritoneal covering for uterine and adnexal structures, transmits the uterine artery through its base (parametrium), contains the ovarian vessels in its lateral extent (IP ligament), and creates the anterior and posterior culdesacs.
The broad ligament is the surgical landmark for hysterectomy — the uterine artery is ligated within its base, the ureter is identified crossing beneath the uterine artery at this level, and the round and IP ligaments are divided within it. Broad ligament haematoma from uterine artery injury or caesarean section complication produces a retroperitoneal collection requiring surgical drainage.
