Proximal Anterior Tibiofibular Ligament

The proximal anterior tibiofibular ligament stabilises the proximal tibiofibular joint. Its disruption occurs in Maisonneuve fractures and in direct fibular head trauma. Proximal tibiofibular joint instability produces lateral knee-to-upper ankle pain on common peroneal nerve territory and may cause peroneal nerve dysfunction from traction.

Anterior restraint to fibular rotation at the proximal tibiofibular joint; maintains fibular head-tibial relationship; supplementary to the proximal tibiofibular joint capsule.

Clinical Relevance

Clinical Notes

The proximal anterior tibiofibular ligament tear is identified on MRI as periarticular signal change at the fibular head. Symptomatic proximal tibiofibular instability may require surgical stabilisation with Kirschner wire or screw fixation.
