The AITFL has two components: the main (Chaput) band and the accessory inferior band (Bassett ligament). The accessory band runs parallel and distal to the main AITFL and may impinge on the anterolateral talar dome during dorsiflexion.
The main AITFL provides the primary anterior syndesmotic constraint. The accessory inferior band (Bassett ligament) lies distal to the main ligament and may articulate with the anterolateral talar dome.
Bassett ligament impingement on the anterolateral talus during dorsiflexion produces anterior ankle pain after ankle sprains — often misdiagnosed as chronic ankle instability. Arthroscopic resection of the accessory ligament relieves the impingement and restores pain-free dorsiflexion.
Anterolateral ankle pain from Bassett ligament impingement on the talar dome during dorsiflexion after ankle sprain, treated by arthroscopic accessory ligament resection with consistent relief.
